Specialist Nurse Coronary Heart Disease AFC Band 6/7 (37.5 hours per week)Applicants without Non-Medical Prescribing qualification will be appointed as Band 6, becoming a Band 7 on successful completion of the course.The South Tees NHS Foundation Trust is a unique organisation delivering acute and community services care.As an integrated Trust, we provide care that is judged as excellent by our patients.We are currently seeking a dynamic and creative Coronary Heart Disease Specialist Nurse to join our Community Coronary Heart Disease team who provide specialist clinical and education services across the Redcar & Cleveland area of South Tees Community services.If you possess innovation, dynamism, drive and are passionate about patient focused care, this could be the challenge you are looking for.This is an opportunity for someone with an enthusiasm for Coronary Heart Disease and a wish to extend their expertise in this field and be part of a team dedicated to providing excellent care across our community.The candidate will be an autonomous skilled practitioner who will be implementing treatment based on expert clinical practice for people with Coronary Heart Disease and assist other health care professionals providing care in a range of settings. You will need to be confident in delivering education to groups.You will have significant post-registration experience, be a non-medical prescriber and have advanced clinical skills to help you effectively manage a caseload of patients with highly complex needs.
