This role offers a rewarding opportunity to provide support and guidance to people with dementia and their carers; helping tomaintain their independence, improving their sense of well-being, and putting them in more control of their lives.This role supports people after their diagnosis in the community or in their homes, where we interact compassionately face to face, by telephone and on-line to give people the tools and knowledge to make informed decisions about their future.In this role, you will develop expert dementia knowledge to provide information and support to those affect by dementia. Based in the community, you will be part of a friendly and dynamic team who provide, face to face services to support the individual needsof our service users. You will manage referrals and your case load; assisting clients in assessing their information and support needs in a person-centred manner is also key to this roleAbout you

  • Working experience of assessing people face to face with an ability to assess their needs.
  • Deliver advice and guidance with a non-judgmental approach
  • Strong organisational and time management skills to manage your caseload of clients effectively.
  • Able to travel independently around North-East Lincolnshire.
  • An understanding of dementia and the needs of those living with dementia and their carers would be an advantage but if you have had experience supporting with other challenges this would be taken into consideration.
  • It is desirable that you have knowledge and experience of Adult and Children Safeguarding, Information Governance policies and procedures and statutory laws such as the Mental Capacity Act.
  • Have a commitment to your continued personal development and learning

  • Be passionate about making a positive difference in the lives of those affected by dementia.
  • Demonstrate optimism, be engaging, and be a pragmatic individual.
  • Excellent interpersonal skills, being approachable, empathetic, non-judgemental with a commitment to inclusion and equality.
  • Excellent communication skills, both verbal and written.
  • Great organisational skills to manage your caseload of clients effectively.
  • Excellent IT skills and ability to use Microsoft packages confidently.
  • Be able to work independently and collaboratively.
