This role offers a rewarding opportunity to provide support and guidance to people with dementia and their carers; helping tomaintain their independence, improving their sense of well-being, and putting them in more control of their lives.This role supports people after their diagnosis in the community or in their homes, where we interact compassionately face to face, by telephone and on-line to give people the tools and knowledge to make informed decisions about their future.In this role, you will develop expert dementia knowledge to provide information and support to those affect by dementia. Based in the community, you will be part of a friendly and dynamic team who provide, face to face services to support the individual needsof our service users.
