Community Mental Health Navigator North East Lincolnshire Your roleAn exciting opportunity has arisen for 3 of the right individuals to join our client as Community Mental Health Navigators. You will be part of an initiative to create a proven new model of care to improve the lives of people severely affected by mental illnessin the UK. Based in North East Lincolnshire (NEL), the Community Mental Health Navigator will aim to complement the community mental health transformation programme and produce positive outcomes and impact for service users.Working with a provider of adult and older adult mental health services and primary care networks in NEL, you will be a key member of the newly integrated community mental health service offer.Your role will be a 2 year fixed term position working full time, 37.5 hours per week based across primary care settings and the local community. You will work Monday to Friday 09:00am -17:00pm, however some flexibility is required depending on service need.What will you be doingReporting in to the Primary Care Mental Health Liaison Practitioner, you will be fully integrated into a multi-disciplinary team working alongside mental health and primary care professionals to support service users with personalised support, and most importantlydelivering great outcomes for people using services. You will proactively engage with service users, identify non-clinical needs and provide the relevant support and guidance required to improve the patients mental wellbeing and quality of life.This can include,- Working as part of an integrated mental health team, with a range of other mental health and primary care professionals to ensure the most appropriate care is provided to service users- Working with partner organisations including the voluntary sector to establish pathways in to support services to meet the holistic needs of your service users- Working with colleagues to ensure friends and families of service users are provided with all relevant information and support required to maintain support for the service userDo you have what our client is looking for- A positive and professional approach to providing mental health support- A passion for delivering high quality care in the local community- Ability to work collaboratively with a range of professionals- A solution focused approach with excellent communication skillsThe post holder will possess:- QCF / NVQ level 3 in Health & Social Care, or formal qualification relevant to the community based Mental Health Care Navigator role- A full UK driving license and have access to own transport- Knowledge of debts, housing, mental health, criminal justice and/or substance misuse- Working knowledge and understanding of mental health and the ability to recognize the indicators of deteriorating mental health- Previous experience in managing a caseload of people with complex needs- Previous experience in a comparable role- An understanding of the local community assets in north east LincolnshireWhat will you receive - Employer funded pension- Flexible working- Life assurance - BUPA employee assistance - Eyecare vouchers- Training opportunities - 25 days annual leave, rising to 30, plus bank holidays - 1 days additional leave (pro rata) on your birthday Webrecruit and our clients are equal opportunities employers, value diversity and are strongly committed to providing equal employment opportunities for all employees and all applicants for employment.
