As a Band 6 nurse you will be responsible for delivering a high quality specialised service, providing expert clinical intervention, education & support within the South Sector Rehabilitation Service to patients and their carersThis will include professional and multi-disciplinary patient assessment, treatment and discharge from care. You will support the rehabilitation of patients in the community and those being discharged from hospital. Avoidance of unnecessary admission to hospital is a key part of our service through urgent referrals from GP’s & Emergency Dept. You will take an active role in clinical effectiveness & service development. You will be involved in the supervision of junior colleagues and students. Experience in Neurology / Orthopaedics / Medical or Care of the Elderly setting is preferable but not essential. Experience of working in the community is desirable but not essential. It is essential that you have a current driving licence. You will hold a Degree/Diploma or equivalent in Adult Nursing and will be registered with the NMC.The shift pattern for this post is between Monday – Sunday 8.30am – 4.30pm.Informal Contact: Donna Mooney, Team Lead.
