A fantastic opportunity has arisen for an efficient and organised Care Coordinator to join our care company.Our vacancy for a Care Coordinator is to support the delivery of the highest quality care services by supporting care workers and be the main point of contact for service users and external organisations.Care experience is essential for this role but wither you are working in care at present or may have left the care sector and looking to return, no matter how long your break, we will provide full training to all successful candidates to the highest standard.Due to internal travel and the field based elements of the role you must be a driver with your own vehicle.If you are passionate about providing the best in community care, take pride in building relationships with your fellow care workers and services users then this role is for you!Typical Duties: Conduct service users reviews and risk assessments Spot checks and supervisions of community based staff Liaise as necessary with partner professionals and organisations (e.g. social workers, local councils, GPS, nurses) in respect of developing and delivering holistic, person-centred packages of care Carry out on-call coordinating and duties outside normal office hours (1 in 3 rota) additional payments out with salary General administration duties Field based shifts (if required)Person Specification: Experience of delivering and managing care support services. Understanding of principles of good care. Able to prioritise and work under pressure. Great IT skills including email and Microsoft office. Flexible; willing and able to work outside normal hours wen requested.This is a full time permanent position, with a fantastic company who promote within and encourage their staff to reach their full potential.
