We have opportunities to join the Royal Devon and Exeter NHS Foundation Trust’s (RD&E) Urgent Community Response teams of front line staff, providing support to patients at home. This includes following discharge from hospital, after illness or surgery and also help with end of life care.The service operates on a 7 day rota system between 07:30-22:00, including evenings, weekends and Bank Holidays.It is essential that you have a kind and caring nature, are patient and able to offer a friendly smile. You must feel comfortable working on your own supported by a team of professional colleagues.Duties will include provision of personal care, clinical duties where delegated, monitoring the patient’s condition and working with health and social care professionals to ensure that patients are safe and well at home. You will need great interpersonal and communication skills and be able to connect with individuals with varying levels of understanding.Location bases include Sidmouth, Seaton and Axminster, Exeter, Exmouth, Tiverton, Cullompton, Honiton, Ottery St Mary, Crediton, and Okehampton. You will be travelling across a wide geographical area linked to one or more of these bases. You must demonstrate willingness and flexibility to work across multiple sites/localities as the need arises.You must have a full UK driver’s licence with access to your own vehicle (with appropriate business insurance) as you will be travelling to patient’s homes to care for them.Please refer to the job description for further details of desirable attributes, skills and qualifications.
