Job Description

Community Care Assistant - Edinburgh Temporary - 36 hours a week, full-time (+3 months)Salary: £16.50 an hour (£30,888 per annum)The Social Care Community Partnership are currently recruiting for a Community Care Assistant across Edinburgh to primarily work with older people and people with learning disabilities.Key responsibilities

  • To carry out non-complex assessments and design and implement Care Plans.
  • To identify and set up appropriate services to meet assessed need, including the provision of equipment, adaptations and packages of care.
  • Undertake non-complex reviews of individuals care plans.
  • Support service user /carers / family in decision making to help promote and maintain service users independence through utilising specific services to ensure successful outcomes for the service user / carer / family, e.g. care at home.
  • To contribute and support Social Workers and Occupational Therapists within a multi-disciplinary team in managing their caseloads by carrying out a range of designated tasks, ensuring departmental objectives and service users needs are met.
  • To undertake basic risk assessments and have an awareness of Adult Support and Protection Procedures.
  • To ensure appropriate and accurate records relating to service users and work undertaken are maintained in accordance with the Data Protection Act and Freedom of Information Legislation.
  • To escort and transport clients as required.
  • To assess, provide and check equipment/ adaptations for service users with disabilities.
Qualifications and experience:
  • Experience of completing needs assessments and delivering and reviewing care plans.
  • Understanding of personalisation and self-directed support.
  • Car users desirable but not essential.
  • A relevant qualification such as: HNC in Social Care/Social Services or SVQ 3 in Health and Social Care/Social Services and Healthcare.
  • Degree in Social Work, Occupational Therapy, Nursing, Community Education or similar qualification.
