Job Description

Domus are currently recruiting on behalf of a highly reputable national care provider, who are looking for a Support Co-ordinator in St Helens. The role is to provide individualised personalised support to people with a learning disability and complex health care needs who are living in shared supported living accommodation in the Clockface area of St Helens. You will lead, coordinate and supervise a team of support workers to achieve the highest standards of practice, customer care and positive outcomes.This provider is a national non-for-profit organisation, who provide support across the country to individuals with Learning Disabilities, Mental Health, Elderly & Dementia. They were established almost 25 years ago, and have had a growing a presence in recent times in providing high quality of care to the residents they support.Key Responsibilities of a Support Co-ordinator

  • To ensure all service users have a designated key worker with identified responsibilities for implementing and reviewing their support plan, working effectively with their housing provider/other agencies and documenting outcomes.
  • To ensure that service users receive a consistent, reliable and personalised service by effective rota management and appropriate allocations when deploying staff to meet their needs and preferences in accordance with agreed support plans and activity programmes.
  • To ensure that commissioned support hours are provided reliably in accordance with the services commissioned hours.
  • To organise rotas as efficiently and effectively as possible and to arrange cover for absences and additional requirements as needed.
  • To ensure that an outcome focused personal support plan and person-centred plan is devised in response to identified needs/preferences, risks and goals of individuals and that an agreed programme of support is put in place.
  • Support Co-ordinator Requirements:
  • At least two years’ experience of supporting people with a learning disability.
  • Health and Social Care Diploma or NVQ 3/4 or higher professional qualification.
  • Experience of supervising staff or coordinating service delivery.
  • Good organisational skills and the ability to prioritise & manage a busy workload.
  • To have a clean driving licence and access to a car
  • Willingness to work flexible hours according to the needs of the project. This will include evening and weekend working as required.
