Job Description

We are looking for a dynamic, passionate and proactive individual with excellent communication skills to help develop work with Age UK (Durham) to develop a new service for Older People in Durham and Darlington.This post has arisen from a successful funding bid to work with Age UK to provide support for Older People who are experiencing such issues as loneliness, isolation, lack of motivation and low mood in response to the on-going Covid Pandemic.The post is a Band 4 Associate Nurse Practitioner/Physiotherapist Associate practitioner/Occupational Therapist Associate Practitioner to join a new team that is being set up in partnership with Age UK (Durham) Professional support to the current Care Home Liaison service. We are seeking a highly skilled and knowledgeable team-player who is able to think holistically and creatively in order to deliver interventions for people in their own homes or via telephone and video support. This is your chance to make a real difference to people experiencing mental health problems and associated distress in response to the Covid pandemic.The successful applicant will need a Foundation Degree in Health and Social Care and Literacy and Numeracy Level 2 or equivalent to be recruited in to this Band 4 position.This collaborative, multi-disciplinary service will work with Age UK (Durham) Community support staff to provide wellbeing and psychologically informed interventions such as Exercise, Occupation, Anxiety Management, Sleep, Graded exposure.The overall aim is to:

  • Improve wellbeing of older people in response to Covid Pandemic
  • Provide low level interventions and support for people to prevent the need for secondary mental health involvement
  • Provide a rapid route into secondary mental health services for people who are experiencing mental health crisis
  • We are committed to the health and wellbeing of our staff and have a number of initiatives and support for staff including access to psychology and counselling services. We are keen to support flexible patterns of working and able to accommodate full or part time hours. The work base will be at Age UK Durham office but some home working may also be required.The service is being managed by Age UK (Durham) with TEWV working in partnership with Age UK (Durham). TEWV will provide clinical supervision. Consideration would be given to a secondment opportunity for staff in the Trust.
