Job Description

Asanintegralmemberofthemulti-disciplinary team youwillworkinpartnershipdemonstrating compassionate caring behaviourstofulfilthe key functions ofthe Community Mental Health Team(CMHT).In particularyouwillberesponsible forthe assessment, formulation,implementationandevaluationofcomplex programmes of care using a strengthbasedrecoveryfocusedapproachtocare that proactively involvesthepersonandoptimisesself-management, and willundertakedutieswithout direct supervisionaswellassupportingandsupervisingwork undertaken by registered andunregisterednursingstaff.You will also beresponsibleforproactivelypromotingpartnershipworkingwithpatients,carersandotherprofessionals/agencies inlinewith legislative frameworks, agreedstandardsand clinical policies; this involves workingin a variety ofhome/neartohomecommunityenvironments.Supervisingthepracticeofothers, monitoring undertakingmentorship/preceptorshipresponsibilitiesand supporting the team leaderintheongoinglearninganddevelopmentofthestaffteamandtheCMHT key functions areimportantaspectsofthisrole, as well as providing support and guidance to GP and other healthcare staff within this role assisting in them to manage their patients mental health and well being.It is essential you are a RegisteredMentalHealthNurse, educatedtominimumdegreelevel/or equivalent. Completionofan evidence basedpsychologicaltherapies/psychologicalapproachesto care formal programme ofstudy would be beneficial.
