Job Description

We are looking for an experienced Head of Service to oversee our Home First service which launched in May 2023. Home First brings together many of our teams to improve quality of care for our patients.Home First was set up by several local authorities and NHS trusts in South Yorkshire and is made up of:

  • A specialist falls service supporting patients at home.
  • An enhanced care home team who will support residents in care homes with their physical and mental health needs.
  • A re-enablement team who will deliver rehabilitation in patients’ homes.
  • Rehabilitation beds for patients who don’t need to be in an acute medical care bed but still need stepped down hospital care.
  • A therapy hub which will help patients keep their independence and quality of life.
  • A virtual ward which will deliver face to face and digital monitoring in the home.
  • An urgent community response team who will provide urgent care to patients in their homes.
Role Summary:To take a key leadership and management role within the Adults Social Care function and as a member of the Adults, Wellbeing and Culture Extended Directorate Leadership Team. You will support and sometimes stand in for the Assistant Director to achieve thegoals of the directorate in alignment with corporate plans, strategies and policies.Duties and Responsibilities:
  • Help Doncaster people and communities achieve positive outcomes from information, advice and support that can help them maintain their independence, safety and wellbeing.
  • Lead, manage, support and guide staff in the service area to foster a culture of learning and improvement.
  • Manage the resources of the service area efficiently and effectively, staying within budget limits but also exploring ways to generate external income that can benefit Doncaster people, such as using technology appropriately. Ensure the service area followsa strength-based approach that respects people and communities as the experts of their own lives.
  • Support the wellbeing of people and communities served by the service area and facilitate a preventive approach that minimises or delays the loss of independence and optimises the use of Council resources.
  • Lead operational teams within Occupational Therapy, Housing Adaptation, Hospital Social Work, STEPS and Positive STEPS and Provision of Assistive Technology.
Requirements:
  • A relevant professional degree or certification in one or more of the services we offer
  • Knowledge and skills in leadership and management, including quality improvement, organisational change and staff motivation, and evidence of ongoing learning in this area
  • A history of high performance as a senior manager in a large and complex organisation
  • Experience of creating, leading, managing and developing teams in a complex organisation
  • Significant involvement in planning, managing and overseeing large and complex budgets
  • Demonstrable achievement in successfully leading high-profile programmes and projects
  • Experience of leading organisational and service change in a challenging public context
