Job Description

An exciting opportunity has arisen for a Registered Nurse to join the community based multi-disciplinary resource team based within the Eastern Vale Cluster area.Skilled in clinical patient assessments, you will;

  • promote the independence of patients/clients living at home/within the care sector
  • Avoid inappropriate hospital admissions
  • work across all health and social care boundaries to coordinate care for older adults with complex care needs.
  • About the role:You will manage a caseload of patients from the GP Practices based within the Cluster and will be responsible for providing Holistic Nursing Care alongside the Multi-Disciplinary Team, District Nurses, GPs and other Health and Social Care Agencies.The primary responsibilities of the role are to;
  • Collaborate with GP/Hospital Consultants and the Vale Community Resource Service to plan and implement treatment for acute illness/injury/chronic illness as required
  • Monitor prescribed medication and act upon findings or where appropriate liaising with medical prescriber
  • Monitor indicators of chronic disease, anticipating possible decline and proactively managing this to enhance well-being, and maintain independence
  • Plan interventions in light of patients expressed wishes and goals of care, facilitating discussion with the family and significant others concerning end of life care plans
  • Plans the assessment of people’s health and wellbeing needs and support clients into other intermediate care or acute sector facilities, where they are unable to be supported at home which will always be the preferred option.
  • About you:You will have demonstrable experience of working with patients with chronic conditions, rehabilitation and/or elderly care.
