Social Worker required for work within the Hospital Team for Cornwall in the Plymouth area. £28 ph. 37 hours a week. Adults Social worker required for 3 months +. Lovely supportive team in Derriford. You will be responsible for Discharge Care Packages, developing and maintain links with other multi disciplinary teams within the Hospital setting. You will manage statutory requirements under the Community Care Act, Mental Health Act and Chronically Sick & Disabled Act. This is a fast paced team whos role is key within the hospital services to ensuring patients can be discharged on time and with the correct care provisions in place. Applicants must be able to adhere to strict deadlines and be confident in their assessment skills and putting care packages together with little notice. You will need a recognised social care qualification and be able to reside and work in the UK.
