Contract Type: Fixed TermLocation: DerbyshireSalary: £20,872 - £22,143 per annum (depending on skills and experience)Working Hours: 35 per weekClosing Date: 04 August 2022Interview Date: TBCContract: This is a fixed term contract opportunity until March 2023.Location: Due to the nature of this role, you will be required to travel independently across all of Derbyshire, as a Community-based worker. This is an amazing opportunity to have a rewarding career providing support and guidance to people with dementia and their carers.As a Dementia Support Worker, you will be offering a vital and compassionate advice service. You will provide community based, face to face services for an increased level of support for more complex cases. Managing referrals, assisting clients in assessingtheir information and support needs in a person-centred manner.We strive to help people to maintain independence, improving their sense of well-being, and putting people in more control of their own lives. We promote a person-centred service so that our support is unique to each individual, based upon their own aspirationsand needs. You will develop a proactive approach in reaching people with dementia and carers who may not otherwise access our services.A CRC disclosure will also be required.About you

  • Be approachable and knowledgeable as a Dementia Support Worker,
  • Ability to deliver advice and guidance with a non-judgmental approach and demonstrate compassion to those affected by dementia.
  • An ability to provide a person centred assessment of needs.
  • A clear understanding of the need for client confidentially.
  • Strong organisational and time management skills.
  • Experience of record keeping and report writing.
Person Specification
  • Be passionate about making a positive difference in the lives of those affected by dementia.
  • Demonstrate optimism, be engaging, and be a pragmatic individual.
  • Excellent interpersonal skills, being approachable and friendly.
  • Excellent communication skills, both verbal and written.
  • Excellent IT skills and ability to use Microsoft packages confidently.
  • Be able to work collaboratively with other team members if/when required.
You may have experience of the following: Dementia Adviser, Care Assistant, Caseworker, Special Needs Assistant, Social Worker, Social Care, Community Care, Helpline Coordinator, Volunteering Officer, Support Worker, Mental Health, Dementia, etc.Ref: 134 951
