Job Title: Care Planner/CoordinatorLocation: Dartford Hours: Up to 28 hours a week hours, days and times to be agreed (plus on-call expectations)Salary: £22,000 - £23,500 (salary is dependent on experience and will be pro ratad depending on hours contracted)Any Care calls attended during office hours 9-5pm will be considered to be part of the role and covered by the salary indicated. As part of the role the successful candidate will be expected to be able to deliver care in an emergency (full training willbe given). If care calls are attended between 7-9am or 5-10pm, payment will be made for each visit between these times at the Carer rate.
