Job Description

We currently have an opportunity for an experienced Support Worker to join our team at Pathwaze Ltd who will be responsible for providing support to our customers across Surrey & South London. PurposeTo provide support to Pathwaze Ltd Service Users in line with the companys aim to deliver a high quality, reliable support service tailored to suit each service users Support Plan. Enabling them to lead an excellent quality of life and develop the abilityto move on to live independently.Roles and Responsibilities

  • Engage 20 plus Service Users for an average of 8 hours of support monthly via group workshops and one on one sessions.
  • Conduct Risk Assessments for potential new Service Users
  • Responsible for the overall wellbeing of assigned Service Users
  • Conduct support sessions via a creative and engaging ways by utilising either property visits or company hub.
  • Ensure approved and signed Care plans are submitted to management within 5 days of new service user moving in
  • Create Support Plans for new service users ensuring that they are written with realistic actions and dates that are achievable
  • To maintain records (e.g. support plans) where necessary, ensuring these are wholly accurate, up-to-date and completed in a timely manner.
  • Create diverse types of workshops for the benefit of our service
REQUIRED SKILLS
  • At least 2 years experience
  • Genuine interest in helping others
  • Ability to clearly and effectively communicate with diverse types of personalities
  • Creative - Organise unique ways to engage service users
