Job Description

Service Coordinator / Homelessness Support Worker Cheshire East - North or South £20,400Our client has an opportunity to work with their Cheshire East Floating Support team to deliver high quality housing and support. Youll work with vulnerable customers living in their own accommodation in either the North or South of the Cheshire East borough.Theyll be interviewing as they go for this position and will close the application process once theyve found the right person.Job requirements

  • Process customer referrals for the floating support service. This includes writing detailed risk assessments with the customer and evaluating their immediate and long-term needs. This also includes making sure that a formalised induction, support and exitpathway plan is created in conjunction with the customer, within the first four weeks in service.
  • Support customers in both areas of their service - resettlement and sustainment. Resettlement offers a six-week support plan to help customers settle back into the community after a temporary stay in supported homeless accommodation. Sustainment is morecomplex, longer-term support for customers who are at risk of losing their tenancy and becoming homeless.
  • Work with the local housing authority to make sure they are aware that your customers have been assessed, accepted and supported.
  • Undertake case management responsibilities, including setting up customers and recording all support on the appropriate internal case management system.
  • Lead confidential, person-centred support meetings with customers on your caseload in a setting which is safe, suitable and accessible for their needs.
  • Represent Concrete and provide reports for multi-agency or professionals meetings about customers on your caseload.
  • Complete accurate records of concern/formal safeguarding reports internally and externally if any customers display concerning behaviours.
What theyre looking for
  • GCSE standard of education with A-C grades in English and maths.
  • Six months paid or voluntary experience supporting homeless or vulnerable people.
  • Someone with an up-to-date awareness and understanding of current housing and homelessness laws, the benefit system and the challenges people without a home face. Youll also be aware of UK GDPR and other data protection requirements.
  • A team player with great communications skills and the ability to develop honest and genuine connections.
  • Someone who is flexible, adaptable and willing to take part in an out-of-hours rota, as required.
  • Great communication and problem-solving skills
  • Someone with a driving licence, access to a vehicle and willing to visit customers in their homes.
  • DBS check.
