Social Worker - Children with Disabilities - Coventry - Up to £29.93 Per HourAn exciting opportunity has arisen to join the Coventry Childrens Disability Team.The social worker should have experience in working with children and families where there is a diagnosed disability or significant health need. The social worke role involves completing assessments to consider risk and need and in formulating care plans which are inclusive of the childrens holistic needs and ensuring multi- agency working and responses is vital.The team work with children and young people and their families from 0 to 18 years of age providing short breaks, support to children subject to Child In Need Plans, Child Protection and also Looked After Children including court work. You will work closely with adult colleagues in terms of preparing young people and their families for adulthood and services post 18 through transition planning.Benefits:
