Job Title: Community PhysiotherapistGrade: Band 6 (potential for a band 7)Trust: Coventry and Warwickshire Partnership NHS TrustLocation: Newfield House, CV14NZWard/Dept: Planned TherapiesHours: From 30h/week, Monday to Friday, 08.30-16.30 (minimum three days)Pay rate: £20.76End date: 3 months from start (subject to change)Would you like to be part of a team whose core purpose is to make a positive difference to the health and wellbeing of people and communities who need services Are you highly motivated and committed with excellent attention to detail and strong communicationIf so, Coventry & Warwickshire are looking to recruit a dynamic, passionate, and experienced community physiotherapist to join their team on a long-term placement.The postholder will assess and treat patients in their own homes with a variety of conditions including neurological, orthopaedic, musculoskeletal, frailty, care of the elderly, management of long-term conditions, respiratory and falls. Interventions willbe patient centres focusing on specific goals with the patients completing onward referrals as required. The postholder will work as part of an MDT with OTs and therapy assistants with clinical supervision regularly provided to support with caseload management.High levels of both written and oral communication are essential to this post with the ability to deliver or discuss difficult diagnosis and prognosis with tact, empathy, managing distressed or aggressive patients or carers/relatives. The post holder mustbe able to communicate accurate and sensitive information to patients, carers, and relatives as well as within their own team & colleagues. Written reports and discharge letters to GPs are also a requirement of the role.Essential Requirements:
