Job Description

We are looking to recruit a Care Coordinator to work within the Chesterfield Integrated Community Services.This post is part of an exciting development to enhance integrated care delivery in the community and the Care Coordinator is a key member of the Community Support Team (CST) and will be for 21 hours per week and will be based in Dronfield.The purpose of the CST is to coordinate the care of patients within a GP Practice who have been identified as being at risk of their current situation breaking down leading to admission into hospital.The CST consists of a Community Matron, Care Coordinator, GP and Social Worker and other professionals as needed.The role of the Care Coordinator is pivotal to the CST as it will be the interface between service users, carers, primary care, secondary care, community care, social care, mental health, out of hours and voluntary organisations.The Care Coordinators are an administrative non clinical role, although applications are welcome from a wide range of experience of staff. We are looking for excellent communicators who have the energy, enthusiasm and motivation to improve people’s lives.You will:

  • Be proactive in identifying patients through admissions and discharge data
  • Be proactive in supporting the CST
  • Follow through actions identified by the CST
  • Follow through with service users and others involved to ensure all services/care arrangements are in place
  • Manage the meetings process, including taking and distributing minutes of meeting
  • You will have:
  • A minimum of 2 years in health or social care profession
  • A proven record of excellent written and verbal communication skills and interpersonal skills
  • Be creative and tenacious in finding solutions to difficult problems
