Job Description

Location: We are looking for Dementia Advisers across South West & North East Essex on an exciting Hospital discharge project.Hours: Full and Part time hours available.Contract: Fixed term contract for 12 Months.We reserve the right to close this vacancy early if we receive sufficient applications for the role. These roles will support people and help provide the tools and knowledge to make informed decisionsabout their future. These particular roles require people who are passionate about getting involved in communities and relish working in challenging environments.These roles will be looking at providing targeted dementia support to people with a diagnosis of Dementia, their friends & families for a successful discharge from hospital to ensure that people have the right support to return to their homes safely and withconfidence.

  • Offering a vital and compassionate advice service to support and guide those affected by dementia.
  • Work on wards within the Hospital environment, alongside Hospital staff for a successful discharge.
  • Provide face to face services for an increased level of support followed by an intensive telephone support after Hospital discharge.
  • Managing referrals, assisting those affected by dementia in assessing their information and support needs in a person-centred manner.
  • Providing knowledgeable and trusted information regarding services available.
  • Signposting patients to a choice of suitable other sources of help, where appropriate.
  • Assessing for CareTech which may support on discharge.

  • Working experience of working with people face to face with an ability to assess their needs.
  • Deliver advice and guidance with a non-judgmental approach and outstanding communication skills.
  • Strong organisational and time management skills to manage your caseload of clients effectively.
  • Experience of working in a hospital or community environment
  • Good IT skills and be able to travel independently across Essex.
  • An understanding of dementia and the needs of those living with dementia and their carers would be an advantage but if you have had experience supporting with other challenges this would be taken into consideration.
