Job Description

Social Work Assistant – Assessment and Case Management – Wales.Are you a passionate, dedicated Social Work Assistant looking for your next role 4Recruitment Services are recruiting a Social Work Assistant to join the Assessment and Case Management Team in the South Glamorgan area.Umbrella Rate: £11.47 - £14.49 Per hour.Hours of Work: Monday – Friday 08:30am – 17:00pm, 30 hours per week.Duration: 3 Months initially.Information of the Role:

  • To support the Learning Disability team case management with reviews, assessment & care planning.
  • To arrange and monitor appropriate services in line with individual care plans.
  • To provide advice and practical help.
  • To liaise with voluntary and statutory agencies.
  • To complete annual care plan reviews and or reassessments.
  • To participate in team meetings and duty rotas.
  • To undertake these duties in line with Council Policies and Procedures.
  • To undertake training and self-development and to contribute to this process in others.
  • To participate actively in supporting the principles and practice of equality of opportunity as stated in the Council’s Equal Opportunities Policy.
  • To work from various locations in accordance with the needs of the role.
Requirements of the Role:
  • NVQ level 3 in Social Care.
  • A Full and Clean Driving Licence including the PCV element.
  • Good understanding of adult social care legislation.
  • Experience of working with older people and Adults with a physical and learning disability through direct contact in a working environment.
  • Ability to work independently and as part of a team.
  • Ability to make relationships with Service users and Carers.
  • Experience of Administrative duties.
  • Experience of working within a hospital setting.
  • Good communication and organisational skills.
  • Understanding of the needs of elderly and or disabled people.
  • (HCC) Appropriate social care training e.g. Mental Capacity Act 2005, Safeguarding, child protection.
