A rare and exciting opportunity has become available in the evolving UHL and St Davids Day Hospital/ECAS services!We are looking to recruit 2 motivated and dynamic nurses with experience of working with older adults in primary or secondary care. The candidates must be dedicated to promoting the independence of patients/clients living at home/within the care sector to prevent hospital admissions. The postholder needs to be an expert generalist with highly developed knowledge whose role involves developing and maintaining a number of service interfaces, aimed at ensuring indiviudals receive the right care, at the right time and in the right place.The post holder will be expected to work within a multidisciplinary team.The unit benefits from;
