An opportunity has arisen to recruit to 2 Neighbourhood Support Officer posts. Funding is currently available until September 2021.The Neighbourhood Support Officer is responsible for ensuring the effective and efficient operation of the Integrated Health and Social Care team by provision of high level administration support of the day to day care planning and service delivery within the defined sector.The post holder will be based in one of five Neighbourhood teams and will promote the sharing and collation and recording of high quality data on the multiagency information systems, whilst developing systems and processes and producing data using a variety of analytical techniques.The post holder will form part of a compliment of Neighbourhood Support Officers across the Bury footprint and will provide support & cover where necessary.The post holder will be required to receive, audit, analyse and communicate a wide range of complex and sensitive information, including confidential service user/patient level information.The post holder will support in the development of person centred multi-disciplinary pathways and integrated care programmes with partners.In line with the Trust’s Single Equality Scheme we welcome applications from everyone irrespective of ethnic origin, disability, age, gender, gender identity, sexual orientation, disability, nationality, religion, marital status, social back ground or trade union membership.All communications to applicants will be via the email address provided on your application form and all further correspondence will be through email, therefore please ensure that you regularly check your emails after the closing date for this post.
