Generalist Social Welfare Advisor (Cardiovascular Social Prescribing Team)Location: Bromley by Bow CentreContract Type: PermanentJob Type: Part time, 21 hours per weekSalary: £29,702 per annum pro rata £17,821.20 per annumBenefits: CompetitiveThe Centre is working with East London Cardiovascular Prevention Group (ELoPE) to bring social prescribing into secondary care for Barts Health patients. Social Prescribing is a way of linking people with non-medical support to improve their physical andmental wellbeing, connecting them with their local communities and supporting them to feel more empowered in their lives.This project will address health inequity and support people who would benefit from:• Assistance or support for challenges in the major social determinants of health, including financial, housing and food insecurity.• Being more connected to people in their local community participating in regular fun, physical activity• Support to engage with and access wider and multiple services.• Support with wider social issues e.g. debt, housing problems, employability issues, relationship problems.Social welfare related matters is one of the main reasons people access support from our services.We are therefore seeking a Generalist Advisor who will support the delivery of this new project.The role will provide social welfare advice to people with cardiovascular disease through Barts health. The role will focus on what matters to the individual and take a holistic approach, providing debt, energy, housing, and welfare benefits advice, andsupporting individuals to access a range of services and activities both at the Centre and through local partners.You may also have experience in the following: Support Worker, Charity, Charities, Senior Support Worker, Social Care, Voluntary Sector, Healthcare Assistant, Care Staff, Care Assistant, Community Development, Cardiovascular Support, Care Worker, CommunitySupport Worker, Wellbeing Advisor, Community Connector, Health and Wellbeing Support etc.REF-212 105
