Job Description

Job DescriptionJob Title: Referral and Assessment Manager - Business Development TeamLocation: Remote working with travel and overnight stays around the South and South West covering Sussex, Essex, Kent, London, Hampshire, Devon and Hertfordshire.Rate of pay: £33k - £40k doeFull time permanent - 37.5 hoursClosing Date: 17/2/20Telephone interviews will take place on 25/2/20Are you an experienced Practitioner or Care Manager looking for a new challenging and rewarding role working with a dynamic teamWould you like to join a Company with ambitious growth plansAspirations are a national, specialist supported living provider. We support people with learning disabilities, mental health, physical disabilities and Autism and are presently rated good with CQC.We currently have an exciting opportunity for a motivated and solution focussed individual to join our Business Development Team as a Referral and Assessment Manager. You will be responsible for delivering on new projects in the region, ensuring the project is completed on time whilst providing an outstanding service to adults, enabling them to live in their own home no matter how complex their needs.Main duties include:

  • The management of all new enquiries and referrals made to Aspirations.
  • Completion of Assessments.
  • Support with the implementation of new packages of support.
  • Completion of documentation and reports making necessary recommendations where required.
  • Completion of support proposals.
  • Support with initial care plans and risk assessments.
  • Communication with key stakeholders.
  • Working in partnership with Operational Colleagues.
  • Attendance at meetings in relation to referrals and people we support.
  • Tracking referral and assessment activity on identify reports and systems.
  • Working with commissioners regarding funding agreements.
  • Support with any packages of care that are at risk in order to prevent the breakdown in the package of support
  • The initial recruitment of new staff.
  • The setting up new provisions, ensuring the project is delivered on time and in budget.
  • Skills, experience and qualifications required:
  • You will be a Practitioner or Care Manager who has expertise and relevant experience at operational level within the field of health and social care, specifically Learning Disabilities and/or Mental Health.
  • Experience with supporting individuals identified as part of the Transforming Care Programme.
  • Qualified to Degree level or equivalent in a relevant subject ieRNLD, RNMH, Social Worker, Health and Social Care, Occupational Therapy, Psychology, this list is not exhaustive all relevant subjects will be considered.
  • Understanding and experience of working with legal frameworks to support people with learning disabilities and mental health needs in the community.
  • Previous experience of preparing reports, attending professional meetings, evidence and analysis of outcomes and funding discussions.
  • Outstanding communication skills both written and verbal.
  • Highly flexible and project driven, able to multi-task and prioritise work.
  • A positive, energetic person with a cando’ attitude.
  • Strong IT skills including MS office, Word, Excel and PowerPoint.
  • Current, clean driving licence essential.
  • Willing and able to travel across the South and South West area including Devon and Hampshire
