Band 6 Community Psychiatric NurseDorset Healthcare NHS TrustCMHT Bournemouth East - Kings Park, Boscombe, Bournemouth, BH7 6JEMonday to Friday 09:00-17:003 Month Contract£30ph WeekdaysKey ResponsibilitiesRegistered professional needed to join the CMHT in Bridport. To undertake the role of care co-ordinator within the CMHT. Work alongside colleagues in assessing and implementing evidence based interventions with clients with an SMI. To risk assess and take ona small caseload for the duration of the period on bank. To attend MDT meetings, support nursing tasks if appropriate, and be prepared to support duty rota.ClinicalEngage in complex health assessments as appropriate and plan relevant care in partnership with services users e.g. Specialist assessment of complex nursing needsUtilising the Care Programme Approach (CPA) and in line with Trust policy, provide specialist care, education and advice to service users (including carers) who are suffering from common, severe and enduring mental health problem to promote recovery and socialinclusion and in accordance with Trust policy act as care co-ordinator as and when required, including adults at risk and supervision proceduresDevelop, implement and evaluate care plans in conjunction with service users, carers and other professionals which reflect individual needsEnsure that everyone involved in the care plan receives a copy and to ensure that all relevant details are recorded on trust IT systemsLiaise with and offer specialist assessment and advice to other agencies and professions. To participate in Multi-Agency meetings to offer advice and opinions to facilitate the on-going treatment and care of clients and to ensure adherence to National ServiceFramework (NSF) guidelinesActively support service users in the management of their medication within a concordance framework and relevant policies. Administer medication, including Intra-muscular injections depot medication as appropriate and monitor side effects. We also offer a £250 bonus for starting this contract this will be paid out to you after 150 hours completed (4 Weeks)If you know anyone who may be interested in the position we can also offer a £250 referral fee
