Job Description

REACT is an innovative service developed by Marie Curie Bradford Hospice and Bradford Teaching Hospitals NHS Foundation Trust. The service aims to prevent unnecessary admission to hospital and reduce inequalities in access to End-of-Life services acrossBradford. The team will compliment and work collaboratively with existing providers across the locality improving the experience for patients and their families.You will also get a very rewarding salary package, with a signing-on lump sum bonus of £2,000 (pro rata), plus a retention bonus of 10% of your annual salary until June 2025 paid after completing 8 weeks in post and induction training.You will be contracted for 30 hours per week.As a Clinical Nurse Specialist, you will be part of a friendly, professional, supportive team, delivering specialist palliative care to the Bradford community. The role is primarily in the community, supporting up to 72 hours for a caseload of patients dischargedby the REACT team working in the Bradford Royal Infirmary Emergency Department.You will be responsible for coordinating a smooth discharge from hospital, providing specialist advice and overseeing quality and clinical practice. You will also be responsible for providing managerial support for a small team of RN / HCAs.As part of a 7 day service operating between 08:00 - 20:30, you will be expected to work flexibly, providing rotational cover for weekends.   • A Registered Nurse with Level 1 registration
  • Recent experience in palliative care
  • Understanding of the specific needs of patients in the End Of Life phase, together with a firm understanding of community working
  • Proficient IT Skills
  • A full driving license with access to a vehicle insured for business use and a willingness to travel
