Job Description

Support WorkerPersonal Support NavigatorOn behalf of my client Im looking for Support Workers to join a Multi Agency Support Team (MAST) as a Personal Support Navigator to proactively work with people in an urgent care setting to raise awareness of alternatives to A&E for people inemotional distress.Support duties will include but not be limited to:

  • Signposting and actively connecting individuals to resources and activities within their community; using an asset based approach, promoting social inclusion and connectivity.
  • Assisting with financial assessments to ensure the individual is receiving their full entitlement. Supporting individuals using direct payments to organise and buy the care and support they have identified they require, assisting them to maximise their resources.
  • Where required, ensuring that needs of individuals are escalated to the appropriate practitioner.
  • Providing a dedicated and trusted point of contact for individuals, carers and families, assisting individuals to contribute to their self care and support
  • Working with the community based team to provide person-centred support within their own environment.
    • Youll have:
  • ability to think innovatively and find creative solutions; involving other organisations and volunteers; being effective as a facilitator rather than a fixer.
  • Committed to using a person centred approach to ensure the personal aims and choices of people are properly represented within the multi-disciplinary team.
  • knowledge of external agencies, who could assist individuals in building social and community networks.
  • An integrated and personalised asset based approach to support and care.
  • Experience of managing caseloads effectively to ensure prompt and effective support.
  • Experience with SystemOne is desirable
