An exciting opportunity has arisen to support a Post-discharge Hospital Team in the West Midlands region as they seek interim support for a rolling 2023 contract.If you are a Qualified Social Worker with experienced working within a Hospital setting then my client is looking to hear from you.What your role will entail:The Team currently provides support to Adults with complex needs with the aim to safeguard vulnerable adults and provide them with information, advice and signpost towards sources of support to help rehabilitate them at home. You will help assess the needs of care for Adults and provide this using all resources at your disposal in order to ensure they are cared for and supported. The service can offer you a competitive rate, up to £32.00 hourly DOE and a stable contract for 2023.You will predominantly be working from home during your contract.
