Job Description

Job Title: Community Nurse PractitionerReporting to: Clinical Lead / Registered ManagerPurpose of RoleTo provide clinical support for a specified caseload of packages, working under the remote supervision of a senior clinical colleague. To provide clinical advice and guidance on the assessment, treatment, management and support of people with a range of complex needs under the direction of senior clinical colleagues. To provide support, guidance, training, education and coaching to others to enable them to undertake their work effectively. To act as an ambassador for Gray Healthcare. To work collaboratively with people, including their loved ones and existing care providers, to identify their needs and devise effective support options for the future. To liaise with commissioners, healthcare professionals and external agencies to ensure that the packages achieve their core aims and objectives.Key Accountabilities

  • Registered Nurse with active and unrestricted NMC PIN
  • Experience supporting people with complex needs (typically at least 6 months post qualifying experience)
  • Understanding of evidence-based practice surrounding people with mental health and / or learning disabilities
  • Clinical knowledge of people with mental health and/or learning disabilities
  • Ability to work independently, with remote supervision from a senior clinical colleague
  • Ability to engage with others positively and proactively
  • Ability to communicate effectively on a 1-1 basis
  • Ability to relate personally and professionally to people we support
  • Ability to motivate and clinically guide others
  • Desirable
  • Educated to degree level
  • Independent Non-Medical Prescriber qualification
  • Experience supporting people in a range of settings, including in acute
