Qualified Social Worker - Vale Community Resource Teamup to £33.39 per hourBarry (home based working role)A Qualified Social Worker is required to join the Vale Community Resource Service based for the Vale of Glamorgan Council, initially until the end of January 2020. This is a full time contract offering a very competitive pay rate of £33.39 for the right candidate.In this role you will be required to support the social work function of the multidisciplinary integrated health and social care reablement team. The team work with people in their own home to maximise functional independence in activities of daily living (ADL), thus reducing the need for admission into hospital and longer term Social Services care services. It also offers therapeutic intervention and reablement support to people following an admission into hospital, so that they can return home as timely as possible. Individuals can receive therapeutic intervention and reablement support for up to 6 weeks. A client centred therapeutic programme / service delivery plan is co-produced with the individual and reviewed throughout the intervention process. The team includes: Occupational Therapists, Social Workers, Physiotherapists, Speech and Language Therapists, Dietitians, Nurses, Reablement Coordinating staff and Reablement Support Workers (Home Carers). There are approximately 65 Reablement Support Workers who work with people in an enabling way to support the individual in achieving confidence and independence following an episode of ill-health. The VCRS Social Workers work with individuals to establish ongoing support needs, linking in with care management teams within Social Services to set up ongoing packages of support if required. The Locality Memory Team is also based within VCRS consisting of an Occupational Therapist, Physiotherapist, Speech and Language Therapist, Dietician, Nurse, Reablement Coordinator and Reablement Support Workers (Home Carers).If you hold a social work qualification and are registered with Social Care Wales, contact Sarah Leigh at Hoop Recruitment based in Cardiff on for more information.
