The Elderly Admission Avoidance Service has been up and running since 2013; the aim of the service is to support the older person at home, reaching into the hospital for rapid assessment and outreaching to the community for on going support and follow up by the primary care services.Opening times 9am to 7pm Monday to Friday, although this may change as the service grows.We take direct referrals from GPs, Community Matrons, District Nurses, Rapid Response teams, Physiotherapists, Nursing homes and from Old age Psychiatric teams. The purpose of the unit is to give direct urgent access to elderly patients who require clinical intervention and to try to avoid a hospital admission. The patient will have had a decline in function and may also need social care input.We require a band 6 nurse with skills and experience in triaging and assessing patients as well as the ability to make decisions/referrals to appropriate services in the community. This person will also have managerial responsibilities and deputise3 for the Band 7 Senior Sister in her absence.
