Senior Keyworker£9.50 per hour (block contract) 25p per mile travel allowanceWe are offering a full-time permanent role to the right person who wishes to take part in our well-established and now expanding Fulfilled Lives programme in the Carmarthenshire Area.Our skilled team support individuals living with different forms of Dementia, mental health and other cognitive impairments.We will nurture talents and skills that you already have that will make such a difference to someone who needs your support!As a Keyworker you will be looking after your own caseload of service users. You will be responsible for assessing their need for any TECH support and the cognitive and physical needs they may have, and you will be putting effective solutions into place.You will receive full training to increase your skills and identify your strengths, so you are able to provide person-centred care for each individual service user, helping to maintain their independence and support them to live well in their own homes.You will be liaising with Multi-Disciplinary Teams to organise the care needed for the service user.Here are some of our staff and user thoughts on our service:In this role, every day is different: I may be reskilling someone to use their cooker one day, and the next, I could be sharing and recording their Life Story, or acting in the role of companion helping to lessen their feelings of isolation; our teambecome extra members of the family, always providing innovative and trusted solutions to everyday problems. Your Requirements:

• Experience in working with Dementia service users
• Current Driving licence and own car is essential
• A good working knowledge of Dementia / Alzheimers and its effects
