Come and join the amazing Amber Valley teams ...... !A new, exciting opportunity has become available for a band 5, Lead Care Co-ordinator, to join our community teams within Amber Valley locality supporting ARCH PCN (Alfreton, Ripley, Crich and Heanor) .We are looking for someone to act as a Lead Co-ordinator for the Care Co-ordinators.The post holder will achieve this by timely communication with team members; co-ordination of service delivery and operational meetings.You will also cover across various GP practices to ensure a consistent service is delivered.The successful candidate will be an excellent role model and provide managerial and leadership support to the care co-ordinators and within the wider PCN framework of GP practices.Key duties will include caseload supervision, appraisals performance and attendance management, recruitment, leading team and service meetings, supporting service development and liaison with service partners.You will need to have passion and drive and a commitment to making a positive difference.For further details / informal visits contact:Mandy Grogan, Integrated Community Manager, ,
